Out of District Request For Transfer Form
Please complete
the form listed above if your student lives outside of the Clarkston
School District boundaries and would like to attend one of the following
schools: Grantham Elementary, Heights Elementary, Highland Elementary,
Parkway Elementary, Lincoln Middle School, Clarkston High School or
Educational Opportunity Center. In
ALL cases in which a non-resident student is released, the student or
the student's parent/guardian shall be solely responsible for
transportation. Forms must be
filed annually by September 15. Once you have completed the
non-resident admission form, please return it to: Clarkston School
District, Attn: Keri Myklebust, 1294 Chestnut Street, Clarkston, WA

Resident
A student who lives within the
boundaries of the Clarkston School District, who would like to attend a
school outside of the district area, will need to complete a
non-resident form provided by the district in which the student would
like to attend. In ALL cases in which a resident student is released,
the student or the student's parent/guardian shall be solely responsible
for transportation. For more information, please refer to Policy No. 3140.
Online Academy Request Form
In the 2013 legislative session, OSPI was
directed to create a standard form to be used by all districts for
Choice Transfers for online school enrollments. (RCW 28A.250.070)
The Standard Choice Transfer System (SCTS) was developed to meet this
need. It is an application within EDS that provides a resident district
the ability to submit a parent request for a Choice Transfer to a
nonresident district, signifying the intent to release the student upon
acceptance. The nonresident district will accept or deny the request
within the system. Email notices are sent automatically from the system
at milestones throughout the process to the parent and the districts.
The system features include the ability to rescind, renew and resubmit
appealed requests.

Declaration of Intent to Provide Home-Based Instruction
A parent who intends to cause his/her
child or children to receive home-based instruction in lieu of
attendance or enrollment in a public school, approved private school, or
an extension program of an approved private school must file an annual
declaration of intent by September 15.
